At Phramongkutklao Hospital in Bangkok, clinical teams have began using daily AI-generated risk predictions to identify inpatients who may benefit from additional screening for carbapenem-resistant Enterobacterales, or CRE. The work is designed around a patient safety premis: can identifying CRE risk earlier help the clinical team act sooner and protect other vulnerable patients? To our knowledge, this is among the first reported real-world uses of AI to guide active surveillance for CRE in a hospital. The deployment is at an early stage, with clinical oversight and ongoing monitoring at its centre.

The European Centre for Disease Prevention and Control has published proposed guidance for establishing and implementing infection prevention and control programmes across Europe. The guidance places healthcare-associated infection surveillance at the centre of effective IPC. It also calls for greater digitalisation, automation and timely access to high-quality data (ECDC, 2026).

Two patients on the same ward, carrying the same organism at the same time, have not necessarily infected each other. That distinction changes what an Infection Prevention and Control (IPC) team should do next, and getting it wrong carries a cost either way.

A predictive model can perform well and still fail to be useful in practice. By identifying infection risks earlier, hospitals can target screening, prioritise investigations and focus prevention where it matters most.

Across four hospitals at Imperial College Healthcare NHS Trust, conventional cluster criteria caught only about 1 in 5 of the patient pairs genomic analysis flagged as linked — and 2/3 of the clusters they did raise weren't genomically related. IPC teams may be missing real transmission while chasing connections that aren't there.

A hospital-wide study from Hong Kong found that targeted decolonisation was associated with a 23% lower adjusted hazard of MRSA appearing in a later clinical specimen. The result is encouraging, but the operational lesson is just as important: identifying an eligible patient is only the beginning. Hospitals also need reliable systems to start treatment, track completion and follow up when the patient has already left.

Infection Prevention & Control (IPC) in hospitals is not held back by a lack of effort or expertise. It is held back by information that is difficult to reach in time. At the same time, many Healthcare-Associated Infections (HAI) are thought to be preventable - clearly, IPC can come to the rescue! For Chang Ho Yoon, Chief Medical Officer at NEX Health Intelligence, Infection Prevention & Control (IPC) teams are already working their hardest to protect patients - but many of them are frustrated, knowing that they could work smarter if only their work environment would let them. That gap between how hard IPC teams work and how effectively they could work is central to what Chang and the wider NEX team are trying to change.

The pandemic showed the cost of having PPE in the wrong place at the wrong time. Hospitals now need better intelligence about when to escalate precautions and when to stop them.
